A patient who plans to enter Korea for medical treatment or recuperation by a local medical institution in ROK, or a caregiver (including family member) of a patient
You are eligible to apply for this visa, if you are a foreign patient who desires to enter the ROK for medical treatment or recuperation without being invited by an medical travel facilitator, and his/her family member as a caregiver
You can apply for a single-entry visa valid for 3 months or a double-entry visa valid for 6 months.
Periods of stay for this visa will be granted for up to 90 days.
① Visa Application Form (Form No. 17), passport, a photo in a standard format, fee
② Documents proving your medical issues such as a record of diagnosis, a doctor’s note issued by a Korean/foreign medical institute
③ Documents proving your reservations or doctor’s appointments in a Korean hospital or a nursing home.
④ A document proving your financial ability to afford the treatment and living expenses during the stay in ROK
⑤ A document proving family relations (Marriage Certificate, Certificate of Family Relation Records, Birth Certificate and others)
* Limited to a patient in need of a spouse and immediate family members to be accompanied
※ The head of Korean diplomatic missions abroad may request for additional documents depending on the condition of the country of residence/jurisdiction if it is deemed necessary for visa evaluation.
You are eligible to apply for this visa if you are a foreign patient or an accompanied caregiver invited by an excellent recruiting organizations designated by the Minister of Justice as an e-visa application agency
* Recruiting organizations are registered as a medical travel facilitator to the Minister of Health and Welfare in accordance with the「Medical Service Act 」
You can apply for a single-entry visa valid for 3 months, a double-entry visa valid for 6 months, or a multiple-entry visa valid for 1 year.
Periods of stay will be granted for up to 90 days.
① Visa Application Form, a copy of passport, a photo in a standard format, fee
② Documents proving the purpose of treatment issued by a medical institute
③ Documents proving a financial ability to afford the treatment and living expenses during the stay in ROK
※ If an inviting organization guarantees a personal identity of an applicant, you are not required to submit this document.
④ A copy of Business Registration Certificate and a copy of Certificate of Medical Travel Facilitator Registration
※ If an institute is registered at Visa Portal, you do not need to submit this document.
⑤ A document proving family relations (Marriage Certificate, Certificate of Family Relation Records, Birth Certificate and others)
※ Only applied to a patient in need of a spouse or an immediate family member to be accompanied with a patient.
【 Proof documents for an accompanying family member 】
A document of family relation confirmation (i.e. marriage certificate, certificate of family relations, or birth certificate) issued by an Embassy in a patient’s country or in ROK
※ Even if an applicant has a family member whose nationality is different from him/her, a document issued by an Embassy in a patient’s country and ROK is also acceptable.
※ If a representative is hired, he/she must submit a Power of Attorney from an applicant, his/her employment certificate and A copy of ID Card.
※ The head of Immigration Office may request for additional documents if it is deemed necessary to review the purpose of entry, validity of an invitation, and qualification of an inviting organization or an invitee.

① Application form for Confirmation of Visa Issuance (Form No. 21), a copy of passport, a photo in a standard format
② Documents proving the purpose of treatment issued by a medical institute
③ Documents proving a financial ability to afford the treatment and living expenses during the stay in ROK
④ A copy of Business Registration Certificate and a copy of Certificate of Medical Travel Facilitator Registration
⑤ Documents proving accompanying family member and caregiver
– Marriage Certificate, Certificate of Family Relation Records, Birth Certificate and others
※ If a representative is hired, he/she must submit a Power of Attorney from an applicant, his/her employment certificate and A copy of ID Card.
※ The head of Immigration Office may request for additional documents if it is deemed necessary to review the purpose of entry, validity of an invitation, and qualification of an inviting organization or an invitee.
